Pat Conkling, CNP, and Cindy Weir, CNP, are relocating to an existing practice here in Rio Rancho, on November 2nd, 2015. They will be moving to the MYO Clinic located at 1790 Grande Blvd. SE, located to the west of US Bank. The MYO Clinic is only about 1 mile from the current Meadowlark Family Healthcare location, so it will be easy to find. The Meadowlark Wellness Center will be relocating there as well. The new phone number is 505-892 0402. The MYO Clinic is a new building, and offers a warm and inviting atmosphere with expanded services.
There will be no changes to your medical care or provider, except for the address and phone number, and there will be specialty services available in cardiac care, and cosmetic treatments with Armin Foghi, MD, PhD.

Meadowlark Family Healthcare is a Family Practice clinic located in Rio Rancho, New Mexico.  We provide a broad range of primary care services, and our practice includes preventive screening and weight loss counseling through our Meadowlark Wellness Center.

We accept most insurance plans and offer a “same day cash discount” for patients who are not insured. Our goal is to promote wellness for our patients from children to adults. We strive to work in partnership with patients in order to achieve their optimal health goals.

Our health affects all aspects of our lives, and we believe that the highest quality of life comes from healthy habits and prevention of illness.
